Case Submission Guidelines 

• Patient Case # (1,2,3)- Participant Name - State Abbreviation

 Required Documents: requires the following (legible) documents for case approval:   

1. Patient Health History and Patient Medical Release Form
2. Diagnostic Quality FMX, Pano, PA of Implant Site, (Optional Kentucky Conebeam or Kansas Cat scan with calipers)
3. Diagnosis and Periodontal Health Status
4. Copy of Full Arch Scans (Export STL File Instructions), or Intraoral photo of Probe measurements 

 Endo Submission Reminder  

• 1 of 3 cases submitted must include a molar endo
• Pre and Post - Xrays are also required
• Click the link to complete Endo Assessment - 80% Pass Rate is required

 Crown Submission Reminder 

• 3 Cases must be submitted
• Pre and Post Xray, Scan, and Intra-Oral Image required
• Use the following STL file instructions to upload your scan
• Click the link to complete Crown Assessment- 80% Pass Rate is required

 Invisalign Submission Reminder 

• Cases started from September 1 - October 31st are only counted as starts
• 1 completed case must be submitted
• Pre and Post Photo Series and a Scan of the completed case are required for submission
• Use the following STL file instructions to upload your scan
• Use the following PPT template to upload your series - note only 1 image file allowed
• Click the link to complete Clear Aligner Assessment - 80% Pass Rate is required

4. Poured Models, Copy of Full Arch Scans (Export STL File Instructions), or Intraoral photo of Probe measurements 

Approval Guidelines: Cases will be approved and ranked based on the surgical difficulty level scale of 1 to 5.

• (Slight to moderate difficulty 1-3) = automatic approvals
• (Above moderate difficulty 4) = Clinical Director approval required
• (High degree of difficulty 5) = will not be approved 

Please allow 7-10 business days for an email response from your mentor.

Course Requirements 

You must register and submit your cases at least 2 weeks prior to the workshop to receive appropriate supplies. 

The following documentation is required for case approval:    Documentation is required for each patient. Cases submitted without proper documentation will be automatically rejected

For each case submission, you must submit a new submission form and include.

• Patient Health History 
• Pano, FMX, BW's, PA of teeth to be treated
• Intraoral Photos 
• Diagnosis, Teeth #'s, and Perio Health Status

Reminder: All procedures must be approved and completed and a post-op endo PA submitted to receive CE Credit.  

The post-op PA needs to be submitted through a separate Submittable portal on the day of the workshop 

Deadlines:      

Cases must be submitted no later than 11:59 PM the day of your workshop to qualify for CE for the Clinical Foundations Workshop (CFW)

PA of completed endodontic case

Am I eligible to participate?

• ALL Aesthetic Continuum Doctors are eligible to participate.
• You may qualify by completing either track: Smile Test Drive (STD) Track or Veneer Case Track
• Only Eligible to win ONE camera during the challenge

Criteria Requirements

Minimum of 10 Smile Test Drives

AND/OR

Minimum of 4 completed veneer cases (case must include 

at least 4 veneers)

Submit between June 29 - November 1

(Workshop Case does not count)

What do I win?

Olympus Mirrorless Camera - Lester Dine Inc.

Important:

Limit of one camera per participant

Cameras will be shipped after December 1, 2026

Veneer qualifiers will also receive an Aesthetic Excellence Award. 

All challenge recipients will be recognized a t Winter Conference

The deadline to submit your case is November 1st

Before submitting your case be sure you have all the required documentation available.  Including a signed Patient Medical Release Form.
